1. Petition. All petitions to terminate a parent-child relationship shall contain the elements required in JuCr 3.3, be verified, and shall state the facts and circumstances which underlie each of the allegations required by RCW 13.34.180 and 13.34.190. Petitions involving voluntary relinquishment of parental rights shall attach the relinquishment rights and consent form signed by the parent(s).

  2. Answer. All petitions to terminate a parent-child relationship and to establish a Chapter13.36 guardianship shall be answered by the parties. All answers shall be in written form and shall conform to the Superior Court Civil Rules/Policy. Failure to answer the petition may constitute grounds for entry of termination or guardianship by default in the Juvenile Court’s discretion.
