Transition Services are activities that prepare students with disabilities to move from school to post-school life.
Now accepting Job Foundation Applications and School to Work Applications
Program Eligibility
Thank you for your interest in Transition Services. Please review the eligibility requirements below to determine which program the student qualifies for, then select that option in your application.
Job Foundation
- Client of Developmental Disabilities Administration (DDA), or presumed DDA eligible and willing to apply
- Born between September 1, 2006 and August 31, 2007 (also accepting applications for younger students that will be kept on file until their eligibility year)
School to Work (S2W)
- Client of DDA, or presumed DDA eligible and willing to apply
- Between the ages of 20–22
- Receive SSI
- Enrolled in Division of Vocational Rehabilitation (DVR)
- Application deadline for 26/27 School Year Cohort: November 30, 2026
Important Note
Students who have completed Job Foundation do not need to submit a separate application for School to Work and you do not have to participated in Job Foundation to be eligible for S2W.

Job Foundation is an opportunity for Transition Students to be able to connect with Employment Specialists earlier (ages 19 to 20 years old). They will work with the student to identify their work readiness strengths, then create a compressive report that is shared with the student's team. This program serves as a foundation for School to Work.

The School-to-Work (S2W) program supports students with developmental disabilities in their third or fourth year of transition services by connecting them to paying jobs and long-term adult services before they exit school. Employment providers, the Department of Vocational Rehabilitation (DVR), the Developmental Disabilities Administration (DDA), and schools work together to develop a person-centered employment plan and support paid job placement. The goal is to strengthen partnerships and increase the number of students who exit school with a job or post-secondary opportunity.
